GENERAL STATEMENT OF DUTIES:
Family advocate for Temporary Assistance to Needy Families (TANF) eligible homeless and at-risk veteran families with children.
ESSENTIAL RESPONSIBILITIES:
Assesses needs, risks, and obstacles applicable to family stability and permanent housing and provides financial assistance for housing stability as needed
Works to make contact with referred clientele families as triaged by the Family Foundations Program Manager
Assists families in developing realistic and achievable housing plans that focus on housing stability and include measurable action steps and timetables
Documents and records all significant interventions, action steps, progress reports, utilization of referral services
Assists families with SCDJFS applications for hard and soft services
Applies Housing First, Motivational Interviewing, and Trauma Informed Care in service delivery approach
Ensures case files are organized, meets eligibility, and documentation requirements are in compliance with funding requirements
Works closely with Program Manager to ensure proper collection of documentation and data
Inputs required data for the HMIS database
Maintain contacts with all child and parent-serving agencies and homeless provider systems. Attends appropriate collaborative networks as needed
Coordinates with all veteran agencies and systems. Attends appropriate collaborative networks
Travels to schools and community resources for homeless and at-risk families to ensure effective local outreach
Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
Other duties as assigned.
RequirementsQUALIFICATIONS:
Ability to collaborate with parents, teachers and other service providers to optimize youths' adjustment and progress.
Ability to model open communication, fair and equitable treatment, and conflict management skills.
Demonstrated supervisory, organizational, interpersonal, and communication skills
Willingness to learn and apply best practice models as appropriate
Valid Ohio Driver's License and ability to meet agency requirements for driving insurability if applicable; reliable transportation
Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation).
MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: Bachelor's Degree in human services or related field preferred. High School diploma or equivalent required.
MINIMUM EXPERIENCE REQUIREMENTS: Knowledgeable about issues of homelessness, domestic violence, chemical dependency, and mental health. Work experience in non-profit/social services providing services to families with children.
