Position Summary
At Stony Brook Medicine, a Case Manager Clinical Education is a valuable member of our team, who provides training curriculum and programs for new and current staff, supports the financial goals of Stony Brook University Hospital and Care Management, maintains competency and compliance with regulatory standards within scope of Case Management including outcomes management, utilization management and staff management and reporting, and assists in the development and coordination Hospital and Community initiatives and CQI initiatives.
Duties of a Case Manager Clinical Education may include the following but are not limited to:
Designs and implements curricular for Case Management orientation programs and continually provides program development for the department.Acts as a mentor to staff and coordinate department orientation of new staff.Oversees staff performance activities and provides theoretical and practical lessons for staff. Organizes programs, education and competency for staff. Maintains goals to achieve clinical, financial, and utilization goals through effective management, communication and role modeling.Acts as a resource on issues related to utilization, coordination of care, discharge and transitional care, and policies and procedures. Develops performance improvement projects that promotes high quality, cost effective healthcare for the people in the communities served. Works closely with Social Work on program development and organization. Maintains Human Resources management, leadership, quality, and operational management objectives.Organizes department educational meetings. Participates in hospital orientation meetings.Facilitates placement of nursing students in the department. Promotes employee satisfaction, supports staff development, and utilizes the progressive discipline process when appropriate.Other duties as assigned. Weekend and holiday flexibility.Unit coverage.
Qualifications
Required: Licensed Registered Nurse having completed or working on their Bachelors' Degree from an accredited college. At least five years of acute care experience. At least three years Case Management experience. Excel, Microsoft, Teams, PowerPoint
Preferred: Masters. Five or more years Case Management experience. Prior supervisory experience. Certified in Case Management with an accredited organization. Experience precepting new employees. participation in educational and quality programs
