  RN Care Coordinator

  The RN Care Coordinator / Case Manager is responsible for assessing, planning, facilitating and advocating for options and services through a continuum of care from point of contact through discharge on assigned patients.The RN Care Coordinator performs this role in such a manner as to meet the individuals health needs while promoting quality and cost effective outcomes. The positions emphasis will be on care coordination, communication and collaboration with nursing, physicians, departments within the medical center, insurers and post acute service providers to pace the care toward optimal outcomes within the appropriate level of care. The RN Care Coordinator will advocate for the patient and family by identifying and valuing patient choice, spiritual needs, cultural, language and socioeconomic barriers to care transitions. In addition, the RN Care Coordinator will protect confidentiality while striving to achieve high levels of patient satisfaction. The RN Care Coordinator consistently conducts the utilization review process and the discharge planning process as required by hospital policies, standards of practice and Federal and State regulations.

  Responsibilities

  The RN Care Coordinator evaluates the medical necessity of a patients utilization of healthcare services with designated screening criteria upon point of admission and throughout the provision of care in accordance with and in compliance of the Dignity Health Utilization Review Policy.

  The RN Care Coordinator establishes and documents a working DRG on each assigned patient at the time of initial review to estimate a targeted length of stay and anticpate the date of discharge for planning and care coordination purpose

  The RN Care Coordinator completes and documents a discharge planning assessment on those patients identified as high or moderate risk by the designated screening process, or upon the request of a physician, NP, PA or patient representative.

  The RN Care Coordinator facilitates the development of a multidisciplinary discharge plan, engaging other relevant health team members, the patient or patient representative and post acute care providers in accordance with the patients clinical or psychosocial needs, choices and available resources. The RN Care Coordinator initiates then initiates the plan to facilitate a safe and appropriate care transition.

  This position will work closely with all departments at the medical center and the post-acute service providers to streamline the patient transition through the health care system and into the community post discharge.

  The RN Care Coordinator participates regularly in Medical Center, departmental, hospital or team meetings pertinent to the accountable areas, and also participates in performance improvement teams and programs as necessary.

  This position requires the full understanding and active participation in fulfilling the Mission of the Organization. It is expected the RN Care Coordinator will demonstrate behavior consistent with the Core Values of the organization.

  The RN Care Coordinator is accountable for staying current with and seeking knowledge of hospital policies, standards of practice and Federal or State regulations pertaining to their practice.

  In addition, this position considers the population served by the medical center and area clinical integration programs and leads efforts to optimize care coordination across the care continuum. This coordination ensures a plan of care for patients in all stages of health needs.

  Qualifications

  CA Registered Nurse (RN) License required.

  Minimum of 1 year nursing experience in an acute hospital setting.

  Prior Care Coordination experience in a clinical or insurance setting is required.

  Masters Degree in nursing with a concentration in Case Management can serve as a substitute for the experience requirement.

  BSN degree or experience equivalent preferred

  Relevant certification is preferred (CCM, ABQAURP or ACM).

  LA City Fire Card required within 90 days of hire.

  Excellent customer service and presentation skills are a must. Strong interpersonal and written communication skills are essential. Demonstrated ability to apply analytical and problem solving skills. Demonstrated ability to manage multiple tasks or projects effectively. Ability to work independently as needed with a high degree of detail orientation. Ability to work efficiently in a fast-paced environment with changing priorities.

  This position is represented by SEIU-121 and is covered by the terms and conditions of the applicable collective bargaining agreement.

  Pay Range

  $49.45 - $65.08 /hour
