**JOB SUMMARYThe Case Manager will work with individuals and families to provide community prevention programs using the Family Strengthening Framework to help families succeed on a path out of poverty. The Family Strengthening program offers assistance in meeting basic needs and securing services, is flexible in the delivery of services, responds quickly and flexibly to family and community needs, and builds on family and community strengths. This position requires establishing partnerships with other community organizations and serving as the coordinator/liaison for clients to obtain needed services. Have a thorough understanding of case management skills, including encouraging self-determination, developing a housing plan, and providing appropriate referrals and supportive services.
**Specific Case Manager Program Overview:Catholic Charities of Southern Missouri provides services to those who served our country and their families through two programs: Homeless Veterans Reintegration Program (HVRP) and Supportive Services for Veteran Families (SSVF). We work with veterans who are homeless, at risk of homelessness, have limited income, or lack employment. Our highly qualified case management team works with clients to overcome the barriers they are facing to create lasting change and long-term success.

ESSENTIAL FUNCTIONS**
Develop a relationship with clients and their families so as to provide advocacy and direction to ensure the client has the best means to reach self-sufficiency.Provide housing stability case management services for homeless and at-risk homeless individuals and families.Advocate and interact with other service providers, churches, and community organizations on behalf of participants and facilitate referrals.Provide extensive case management, including minimal bi-weekly contact with each client in combination with phone and face-to-face visits in varied settings, including the clients home.Complete written and data-based documentation (i.e., internal documentation case notes, housing plan, etc.)Attending community meetings as determined by the supervisor.Performs other related duties as assigned.KNOWLEDGE, SKILLS, AND ABILITIES
