　　BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Pasco County!

　　Location: New Port Richey

　　Status: Full-Time, Exempt: No

　　Shift Hours: 8:00 AM - 4:30 PM

　　Shift: Days

　　Weekend Work: None

　　On Call: No

　　The Case Manager is responsible for:

　　Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.

　　Coordinating the service delivery system for patients.

　　Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.

　　Performing other duties as assigned.

　　Bachelor's in Related Field, Human Services PreferredExperience:
