This position provides casemanagement services to help individuals live independently in clean,safe homes or apartments to delay institutional placement. The CaseManager works collaboratively with a team of Homemakers serving olderadult clients and adults with disabilities. Conduct the initial in-homeassessments, identifying needs, and developing care plans. Collaboratewith Homemakers to monitor progress, ensure quality of care, problemsolve through challenges, providing support and guidance in accessingother services (such as Medicare, food stamps, etc.) Manage client andhomemaker schedules and appointments to meet client needs.
