The basic function of this position is to provide professional, client-centered guidance and support to individuals who have physical and/or intellectual disabilities, mental health conditions and/or medical care needs. This position provides personal and/or medical cares, models appropriate behaviors, and provides behavior management. Assists with development and/or teaching of daily living skills as written in the Support Plan (SP). Assists people with reaching personal goals, learning new skills and being an active participant in the community. QUALIFICATIONS: Must be 16 years of age or older.* Must have English language proficiency (both written and spoken). Must have a cell phone or device with GPS capability, dependable transportation, a current driver s license, car insurance and a driving record that meets our insurability standards. Must be able to meet all employment requirements, defined by CCRI s policy and procedure manual. Must pass First Aid competency, CPR competency and the CCRI Medication Administration Course (SLS only). Transport individuals (adhering to all safety policies and laws) as needed to meet program objectives and individual needs.* Provide opportunities for new experiences in the community or home. Support new team members by providing peer training. Maintain professional boundaries. Cross-train at multiple sites when requested. Exhibit patience while working with those we support, guardians, and team members. Complete ongoing training as assigned. Prioritize tasks according to need. Serve as a mentor to individuals. Provide guidance and direction while being a positive role model. Assist clients with self-advocacy skills. Advocate for the people you work with in all aspects of their lives. Promote CCRI s Wellness in Action program. Connect clients with our community through volunteerism, events, partnerships, memberships, etc. Daily Living Skills Complete activities with individuals including household chores (cooking, cleaning, laundry, dishes, vacuuming, fixing bed, household maintenance, etc.), personal cares, community involvement, communication skills, personal safety skills, and appropriate behavior skills. The level of assistance a DSP provides varies by a person s individual needs and preferences. Assist with teaching and preparing healthy meal plans to encourage optimal health. Ensure the safety and well-being of the individual(s) served. If applicable, utilize therapeutic intervention techniques with individuals exhibiting verbal or physical aggression. If applicable, assist or perform personal cares bathing, bowel and bladder cares, exercising, range of motion, repositioning, feeding (oral or feeding tube), hair care (brushing, washing, styling, shaving), dental care (brushing, flossing, denture care), nail clipping, etc. When applicable, medication administration following the six rights. Ensure and encourage medical appointment attendance and the following of physician orders and medical protocols. Ensure emergency medical services are provided. Assist with the coordination of Community Residential Setting (CRS) if applicable (SLS only). Documentation Implement and provide feedback for person-centered and behavioral outcomes. Participate in required meetings. Accurately complete, maintain, and submit all documentation as required by CCRI and state and federal regulations that govern us. Complete progress notes/flow charts, service outcomes and support plans according to the individual s SP. Assist with updating training packets, site emergency numbers, possessions lists, team member phone lists, client info sheets, etc. Complete team leader reports; attend team leader meetings as scheduled (SLS team leaders only). PHYSICAL DEMANDS While performing the duties of this job, the team member is required to continuously use hands for simple and firm grasping and fine manipulation. Must continuously be able to manage and cope in sometimes stressful situations. Physical demands will vary for each person we serve.
