JOB DESCRIPTIONJOB TITLE: Care ManagerCATEGORY:Non-ExemptREQUIREMENTS: An individual who is interested in building a cooperative team which will deliver care in a personal, customized, and consistent manner. Ability to assess resident's status and report accurately. Must be at least 18 years of age. CPR certification required, must complete the orientation/educational training for ACLF.

QUALIFICATIONS: Must love working with the elderly. Positive attitude, good customer service skills, ability to work with team members. Requires minimal supervision. Individuals must be willing to foster independence in the resident. Individual will have the ability to assist, cue, encourage, or instruct the resident to perform at his/her highest potential.RESPONSIBILITIES:PHYSICAL1. Assist with ADL's: grooming, bathing, dressing toileting, dining, assistance with walking, transferring, and medication assistance.2. Coaching and cueing of residents, encouraging independence.3. Pertinent record keeping.4. Maintenance of a safe environment for residents/fellow staff.5. Routine vital signs.6. Transportation of residents between sites with ACLF.7. Assist with activities/crafts/offsite trips.8. Perform the universal worker role.9. Light housekeeping/laundry duties.10. Assist the residents in the dining room to include serving meals, setting up and cleaning of the dining room, assistance with meal selection, with a focus on customer service.11. Any other assigned duties as requested by the Wellness Director/Administrator.

REQUIREMENTS: Must be able to walk, stand, bend, and lift a maximum of35 lbs. Free of communicable diseases.
