  Position Summary Information

  Job Description Summary

  The Care Manager will assure that Health Home enrollees receive needed medical, behavioral, and social services with the goal of improving individual health outcomes. Will have overall responsibility and accountability for coordinating all aspects of the individual’s care. Will collaborate with multidisciplinary personnel (i.e. medical, mental health, nurses, etc.) to assure patients receive services as indicated in the enrollee’s plan of care.

  Minimum Qualifications

  Knowledge, Skills and Abilities:

  Must have excellent customer service, interpersonal and communication skills. Broad knowledge of health and community resources. Ability to work independently as well as with supervision and as part of a team. Demonstrated sensitivity toward the client population serviced. Bi-lingual proficiency preferred. Ability to independently research and retrieve information from internal reference material or from outside sources. Demonstrated critical thinking skills.

  Education and Experience:

  Bachelor’s degree or equivalent required. Healthcare education, certification, and/or training required. Two years of care management experience required. EMR Super User preferred.

  Preferred Qualifications

  EMR Super User preferred. Bi-lingual proficiency preferred.

  Physical Demands

  While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. The employee must occasionally lift up to 15 pounds. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus.
