  Overview

  We are looking for a Care Coordinator Non Clinical to join our team. This position will directly report to the Director Contact Center and is responsible for taking non clinical inbound calls and escalation to a nurse, as well as to support referral coordination for patients who need Gentiva Services.

  Able to navigate healthcare options; care services post-acute offerings, Medicare coverage, billing issues, as well as accessing healthcare resources.

  Utilize a variety of tools and methods to quickly provide patient options and education including but not limited to sites of service, specialty offerings, post-acute care, and other related questions.

  Assists in admissions process by working closely with the Gentiva branch staff to follow up on patient admission status.

  The shift is:

  Monday: 5p-1:30a EST

  Tuesday: 5p-1:30a EST

  Thursday: 5p-1:30a EST

  Friday: 5p-1:30a EST

  Saturday: 10a-6:30p EST

  About You

  Associate degree preferred

  Minimum of 1 year working in a healthcare related field or healthcare call center

  INTERNET REQUIREMENT: High-speed Internet connection (minimum 50 Mb/s download speed and minimum 8 Mb/s upload speed, recommended 8 Mb/s upload). Satellite based internet services are not acceptable.

  Excellent analytical and problem-solving skills.

  Ability to learn and master information related to locations and services of client.

  Outstanding computer skills.

  Good time management skills

  Good working knowledge of home health, hospice and palliative care services

  Ability to communicate effectively with empathy over the phone and while interacting with others; excellent interpersonal skills.
