Hennepin Healthcare is currently seeking a Licensed Mental Health Professional to join our Collaborative Care Model team as a Behavioral HealthClinician. This full-time role will primarilywork dayshift on-site on the Minneapolis downtown hospital campusand/or at the Whittier Clinic in South Minneapolis.This is a grant funded position through a five-year grant from the Substance Abuse and Mental Health Services Administration (SAMHSA), in collaboration with the Minnesota Department of Human Services.Successful candidates must be a licensed Mental Health Professional per MN statute – LMFT, LICSW, or LPCC.
SUMMARY
The Behavioral HealthClinician is a core member of the Collaborative Care team, which includes the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team.The Behavioral HealthClinicianis responsible forsupporting and coordinating thebehavioralhealth of patients on an assigned patient caseload with the patient’s medical provider and, whenappropriate, other mental health providers.This role will also systematically track treatment response andmonitorpatients (in personand/or by telephone) for changes in clinical symptoms and treatment side effects or complications.The Behavioral Health Clinicianwill providedirect mental healthand/orsubstance useservices including screening, diagnostic assessment, psychoeducation, and psychotherapy services with persons living withmental illness and/or addiction. The role will provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments asappropriate,participatein regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider.
RESPONSIBILITIES
Participates in multidisciplinary care team rounds Providesmental health services including initial and ongoing screening, diagnostic assessment, treatment planning (including regular updates), psychoeducation, psychotherapy services, crisis services, referrals, and discharge/termination planning Completes necessary documentation foraccurateandtimelybilling of clinical servicesprovided Providespsychoeducation to patients, families,colleagues,and community organizations as needed Providescare coordination including discharge/termination planning Provides clinical supervision to graduate-level mental health practitioners in compliance with HHS and regulatory guidelines (e.g., licensing boards, state regulations, etc.) Assistsinmaintainingstandards of accreditation by health management organizations and state licensure/certification Other duties as assigned QUALIFICATIONS
