Job Description
Directs the care management programs in Northwell Health hospitals. Monitors and evaluates key performance indicators related to clinical process and participates in the development and implementation of financial and strategic planning activities and facilitates quality improvement initiatives. Works with health system leadership to establish and maintain operational infrastructure.
Job Responsibility
Leads a system-wide functional group by developing, communicating, and building consensus for goals/programs that support system goals Develops and articulates a long-term (1+ years) strategic vision for areas of responsibility. Directs the implementation of the transitions of care model, including policies and standards to improve patient care and reduce readmissions. Oversees, evaluates and ensures efficient and effective operational and quality performance, including the development of quality dashboards that reflect mandatory measures in value-based contracts; responsible for reporting performance to Northwell Health site and corporate leadership. Evaluates and directs the assessment of services and development of new programs and research projects on an on-going basis and supports strategies for program adaptation to emerging models and commercial and governmental programs. Directs the implementation of human resources plans to achieve strategic goals and objectives including organizational development, recruiting, compensation, training and development, employee engagement and relations. Selects, develops, manages and evaluates direct reports; and oversees the selection, development, management and evaluation of indirect reports. Ensures compliance with federal, state, and local regulations and tax laws, and regulatory agency standards of compliance. Interprets and standardize fiscal and regulatory reporting standards to ensure compliance with government and regulatory reporting and third-party agencies. Ensures compliance to established disaster response and recovery plans to ensure patient safety and business continuity. Represents Northwell Health to external partners, managed care organizations, and others as needed. Performs related duties as required. Job Qualification
Master's Degree in Nursing or related field required, or equivalent combination of education and related experience. Bachelors or Masters must be in Nursing.
Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
10-12 years of relevant experience and 7+ years of leadership / management experience, required.
