Job Summary: Assists the stroke coordinator to manage and coordinate systems related to acute stroke care delivery. Supports clinical service and enhances quality improvement and research activities in the clinical setting. Acts as clinical liaison between physicians, clinical coordinators and administrative staff. Job Responsibilities: Monitors data related to quality of care issues and pathways for stroke care management including patient education. Reviews process of care metrics, disseminate analyses and data summaries. Establishes system to monitor patient satisfaction. Maintains the structure for Joint Commission Comprehensive Stroke Center recertification. Reviews data abstraction related to stroke metrics and compliance with Joint Commission and AHA standards of care. Develops and maintains stroke order sets and guidelines for care to enhance stroke-related medical and nursing patient documentation in the electronic health record. Develops and maintains community education programs. Prepares education for outreach events, stroke screenings and lectures for local or regional events. Works with AHA and CT Stroke groups to promote stroke awareness within the local community. Develops and maintains staff education programs. Actively participate in stroke conferences and nursing education for stroke care. · Bachelor’s degree in Nursing. Master’s degree in Nursing · Minimum of 3 years’ experience working with Neurology/Neurosurgery patients or patients with stroke. · Current CT RN license.
