About Ambulatory Clinical Documentation Improvement LPN - Full-Time (Up to a $15,000 sign on bonus)

  SUMMARY:

  Dedicated to centralize and coordinate health promotion and risk factor modification activities for the needs of population health. Responsible for creating, organizing, scheduling, and promoting outreach efforts as well as collaborating with clinical and financial leadership to support fulfillment of quality metrics. Complete patient outreach and assist with population health activities as assigned.

  In addition to the duties of the outreach coordinator, the Outreach coordinator Level 2 requires extensive knowledge of coding guidelines to properly reflect severity of patients illness. The Outreach coordinator will work with both the manager of Value Based Reimbursement as well as the Outpatient Clinical Documentation Improvement team to capture opportunities for patient outreach and risk adjustment capture.

  EXPERIENCE:

  Minimum 3 years clinical experience. The Outreach Coordinator Level 2 will have extensive communication with the patient population and will need the ability communicate effectively both verbally and in writing. Must have effective interpersonal and customer service skills, including professional phone etiquette with a positive, nurturing, and flexible attitude. Must be able to interact with all age, social and economic levels. Organize, prioritize, and manage time efficiently to meet established deadlines. Operate PC and other software as required to perform job duties such as preparing reports. Maintain effective interpersonal relations.

  EDUCATION:

  LPN with current license in State where employment is located. Additional coding knowledge or coding certification desirable. Expectation to obtain CPC certification within 1 year of hire.

  ESSENTIAL FUNCTIONS:

  Coordinate activities related to population health initiatives including but not limited to various patient outreach efforts.

  Analyze quality metrics and participate in efforts for population health management, including comprehension of patient records for correlation of appropriate metrics.

  Demonstrates understanding and thorough working knowledge of ICD-10-CM coding guidelines, as well as medical terminology to capture severity of patient illness and final code assignment.

  Educate patients and provide information in a knowledgeable, friendly, and effective manner.

  Must work effectively independently and able to organize, prioritize and manage time efficiently to meet department deadlines.

  Educates members of the team, to include but not limited to physicians, practice managers, and clinical staff, regarding documentation needs, reimbursement issues, and performance improvement opportunities on an ongoing basis.

  Preparation of reports summarizing activities and performance metrics related to process improvement initiatives.

  Participate in process improvement activities as needed.

  Participates in problem resolution activities to help eliminate barriers to patient care.

  Participates in professional development activities and maintains professional affiliations. Maintains confidentiality.
