　　Description and Responsibilities

　　Open Sky’s Adult Family Care supports individuals over the age of 16, elders and adults with and without disabilities who cannot live alone safely. The individuals served in the program are in need of assistance with one or more activities of daily living and are cared for by either family members or non-family members. The AFC program provide caregivers with several supports such as nursing supports, professional case management, clinical consultation/training, and 24-hour emergency support. With these supports in place, we are able to aid in the caregiver’s ability to care for their loved one.

　　The Registered Nurse visits families in their homes within the Central Massachusetts region and operate out of an office in Whitinsville. This is a full time position and is predominantly first shift Monday-Friday with no on-call responsibility. The RN will complete intakes, annual assessments, blood pressure readings and general overall visual assessments to the individuals served in Adult Family Care.

　　Other Key Responsibilities:

　　Maintain necessary contact with medical service providers.

　　Hands-on requirements limited to taking vitals and blood pressure readings, and general overall visual assessment.

　　Upkeep of typical documentation and progress notes in electronic healthcare record (eHana).

　　Provide medical teaching and training to caregivers and persons served, and peers/teammates as needed.

　　Perform medication audits as needed.

　　Qualifications

　　Licensed Registered Nurse in Massachusetts, required.

　　Motivated with ability to work independently and autonomously, required.

　　Valid driver's license, acceptable driving record, and reliable transportation, required.

　　All staff need annual physical and TB screening under MassHealth guidelines.
