　　The Adult Care Manager facilitates a planning process that builds on the patient's strengths, supports and promotes stability, wellbeing and self-sufficiency. Ensures that individuals are informed decision-making participants by establishing and coordinating an interdisciplinary team that includes medical, behavioral health, and other community providers.

　　QUALIFICATIONS:Bachelor's degree in health or social sciences and one year of related experience or, Associates degree in health or social sciences and two years of related experience,Reliable transportation.

　　COMPREHENSIVE CARE MANAGEMENT:Maintain assigned caseload of forty- forty five adult individuals.Complete comprehensive assessment identifying medical, behavioral health, chemical dependency and social needs.Develop a person-centered plan of care that integrates service needs and identifies goals.

　　HEALTH PROMOTION & CARE COORDINATION:Support adherence to treatment recommendations.Link to needed services to support well-being and goals.Advocate on behalf of the patient to ensure all care team members are informed of current treatments and preferences.
