This position will serve as the point of contact for referral sources, families and patients. Position will manage the insurance conversations with patient and families as needed. This position will also serve in the role of obtaining and entering admission physician orders into the Cerner system in a pre-planned state. Reports to Post Acute Executive.RESPONSIBILITIES & EXPECTATIONSDaily collaboration with Hospital Discharge Services and Utilization Management staff members to identify actual and/or potential candidates for post-acute care.Collaboration with Aultman post-acute units and leadership team members.Communication with medical staff and clinical provider(s) as indicated.Review of electronic medical records and clinical documentation for referred patients.Review of evidence-based criteria evaluations specific to level of care.Face-to-face assessments for referred patients (as required).Telephonic and face to face contact with patient, families and/or representatives.Completion of required preadmission screening forms for Inpatient Rehabilitation referrals.Communication with utilization management and/or specific payer sources regarding pre-authorization and/or verification of insurance benefits.Documentation in electronic medical record including, but not limited to, Cerner.
