What You Will Do:
Provides screening to identify and problem-solve barriers to treatment engagement and retention. Responsible for assessing and coordinating necessary resources to promote access to treatment.
Coordinates referrals to meet vocational needs, physical health, food, clothing, transportation and shelter needs as identified upon screening.
Renders all services within department guidelines and in compliance with rules/standards set forth by Medicaid/Medicare, ACHC, OhioMHAS, and CARF.Promotes patient recovery through active and collaborative patient involvement in initial intake and ongoing treatment planning. Fosters recovery and independence by identifying and incorporating patients’ strengths and abilities into the treatment process. Involves natural support persons, self-help resources, and other community integration activities as critical components to long-term recovery.
Responsible for employing assertive outreach, motivational interviewing, and other evidence-based practices to promote engagement and retention in behavioral health treatment services. Extensive efforts are made to outreach clients who are identified to be at-risk if not involved in treatment services. Documents all outreach attempts.
Facilitates access to care through identification of and problem-solving barriers that prevent access to treatment and other social services.
Bridges service planning between inpatient and outpatient behavioral health services as appropriate to role.
Assists with gathering demographic information as needed to facilitate access to outpatient behavioral health services.
Coordinates intake appointments with outpatient reception/clerical staff and involves the management team as needed to arrange or expedite clinical intake appointments.
Facilitates access to crisis intervention services as warranted.
What You Will Need:
High school diploma is required, Associate’s degree or higher in social services related field preferred. Experience working with persons with mental health or substance use problems preferred.
A high level of organizational skills is required.
Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
Ability to effectively understand and communicate in both written and oral form. Sensitive to the cultural needs of the individual and/or family served.
Ability to be mobile and possess a valid State of Ohio motor vehicle operator’s license and be insurable under Firelands’ auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
Successful completion of a 180-day probationary period.
