About the Role:
Located at SASH affordable housing site(s) and an active member of theSASH team including the SASH Coordinator and Wellness Nurse (RN), theSASH Clinician provides whole-person care coordination and supporttargeted to individual participants as well as the SASH population as awhole. Specific interventions by the SASH Clinician will be flexible tomeet the needs identified at each panel (up to 100 Participants).
Principal Responsibilities:
- Direct individual therapeutic support provided in-person, by phoneand through telehealth including individual, couples and family therapy,and other psychosocial support
- Assessment, screening, and triage services to help identifyunaddressed needs and to assist individuals in connecting to clinicianand/or other community resources and providers
- Group-based programs on topics relating to mental health, substanceuse, aging, and other emotional needs
- Additional Direct Services may include Conflict mediation sessions;Support with completing Advance Directives and obtaining
- DNR/COLSTs; Support with housing retention
- Care coordination to include regular consulting meetings withembedded SASH staff, monthly SASH team meetings, and outreach to primarycare, psychiatric, and other community partners and service providers
- Provide training and consultation to SASH Coordinators and WellnessNurses addressing challenges confronted by SASH participants with mentalillness, substance misuse and other social, behavioral, or emotionalconcerns
- Develop and present trainings and workshops to agency or SASH staffor to other populations as identified by the team
- Other needs as identified by staff, residents or through directobservation of need
- Utilize SASH data management system (s) in order to collect andpresent outcome data for the program
Qualifications and Competencies:
- Master\'s Degree in counseling, social work, psychology, or closelyrelated field
- Licensed OR on the roster of Non-licensed Non-certifiedPsychotherapists
- Excellent interpersonal, communication, problem solving, and conflictresolution skills.
- Able to work independently and as part of a team and exercise matureand professional judgment
- A highly motivated self-starter and ability to coordinate multipleprojects at once.
- Flexible, adaptable and have the capability to work in a fast paced,professional environment
- Ability to enter a stressful situation calmly and professionally andmake appropriate assessments
- Computer literacy - MS Outlook (email, calendar and tasks), MS-Word,MS Internet Explorer software.
- Ability to provide case management with highly vulnerablepopulations, particularly those with health, mental health, andsubstance abuse issues
- Must have a valid driver\'s license, reliable transportation withpersonal automobile liability insurance coverage at or above the levelsmandated by the State of Vermont.
