Community Care Network is comprised of Rutland Mental Health Servicesand Rutland Community Programs. Our mission is to enhance the well-beingof our communities, individuals and families through responsive,innovative and collaborative human service. Through this position, youwill be responsible for overseeing the service coordination forindividuals with developmental disabilities.
About The Role:
We are looking to fill our Service Coordination role on our Home andSpecialized Supports Team. This position is responsible for developingservices, monitoring effectiveness, maintaining required documentationand meeting regulatory and quality guidelines. You will work closelyalongside a team of people to help provide the person-centered service.Ideal candidate will be committed to positive solution based thinking,dedicated to supporting individuals to lead thriving lives and excitedby challenges. We are seeking someone who is willing to help facilitatecommunity inclusion, life skills, positive relationships andself-advocacy.
What You Need to Succeed:
- Bachelor\'s Degree in related field required.
- Previous experience working with individuals with developmentaldisabilities preferred.
- Must have a valid driver\'s license, reliable transportation withpersonal automobile liability insurance coverage at or above the levelsmandated by the State of Vermont
- *Successful completion of a criminal history background check afterhire.
