　　As a Physical Therapist at Champion Rehab, you'll:

　　Provide direct patient care: continuous assessment of patient needs and functional deficits, patient care/treatment, ongoing modifications to the existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed

　　Work closely with a skilled and dedicated interdisciplinary team to maximize patient satisfaction and overall success based on diagnosis and comorbidities

　　Apply clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, supervision of Physical Therapy Assistant, communicating with Physicians to ensure fluency of care

　　Complete documentation which follows regulatory guidelines (state, federal)

　　Qualifications for Physical Therapist:

　　Graduate of an accredited Physical Therapy program

　　Currently licensed to practice in the State of MN. New graduates and temporary license holders are encouraged to apply
