　　Physical Therapist - Home Health Care Services – Sea Crest Home Health and Hospice-Costa Mesa

　　Schedule: Variable Days, Mon-Fri with required Saturday/Sunday rotations, with options to work on weekends if desired

　　Area Coverage: Anaheim, Brea, Buena Park, Fullerton, Garden Grove, Orange, Placentia, Santa Ana, Yorba Linda

　　Job description:

　　Physical Therapists develop, coordinate and maintain therapeutic services and activities alongside an interdisciplinary team to help patients in need successfully maintain or improve their level of independence.

　　Required qualifications:

　　Master's Degree in Physical Therapy or

　　Degree in Physical Therapy from an approved program according to Medicare guidelines.

　　California Physical Therapist License upon hire.

　　National Provider BLS - American Heart Association upon hire.

　　Preferred qualifications:

　　Home Health experience.

　　One (1) year experience as a Physical Therapist.

　　Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
