The Case Management Extender works with Partners for Kids staff, hospital staff, health plans, and community agencies to arrange for and/or connect patients to needed services. The Case Management Extender will identify, create and nurture relationships with local agencies, schools, churches and other programs that can provide services to children and their families. Case Management Extenders are patient and family facing and they interact with them on a daily basis. A Place to Be Proud.ResponsibilitiesThe Case Management Extender (CMX) functions as a member of an interdisciplinary team and provides care coordination services to patients and their families including:Screening and identification of patients (Occasionally)Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination (Frequently)Assisting Care Coordinator with the development of family-centered plan of care (Occasionally)Facilitating communication and collaboration amongst the healthcare team (Frequently)Developing patient's/family self-management skills through education and resource provision (Continuously)Performs ad hoc tasks as directed by Care Coordinator (Frequently)Managing transitions between settings, caregivers and providers (Frequently)Monitoring and follow-up across the continuum of care (Continuously)Participates in orientation and continuing education of staff and students as appropriate (Frequently)QualificationsEDUCATION:Bachelor's degree preferred, with background in health care, public health, or rel
