JOB REQUIREMENTS: JOB PURPOSE AND REPORTING STRUCTURE: Under thedirection of the Youth and Family Supervisor, the Youth and FamilyPsychotherapist provides youth-oriented behavioral health and substanceuse disorder therapy in the behavioral health clinic. The Youth andFamily Psychotherapist provides clinically appropriate care from intaketo discharge, in accordance with agency policies and procedures. Theycoordinate care, facilitating interactions with informal and formalsupports. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.Other duties may be assigned. Completes comprehensive mental healthand/or substance use assessments and establishes service plans Providesevidence-informed individual and/or group therapy Meets all legal,regulatory, and agency standards in providing care Completes requireddocumentation of services, utilizing dedicated electronic healthrecords. Collaborates with behavioral health and primary care providers,families and other formal and informal supports for integration of careFacilitates referrals to other needed services and/or levels of careProvides crisis prevention and intervention as needed Provides on-callduties as assigned Maintains continuing education and licensing, asrequired by WI DSPS Expected to maintain productivity expectations,communicating to request additional referrals when able to accommodatewithin their schedule. Provides limited walk-in clinic coverage,including brief screenings and referrals to appropriate resources.EDUCATION and/or EXPERIENCE: Requires a master\'s degree in a jobrelated major and post-degree state licensure, which may include atraining license (Licensed Professional Counselor, Licensed ClinicalSocial Worker, Licensed Marriage and Family Therapist, LicensedProfessional Counselor-In Training, Advanced Practice Social Worker).Considerations & Statement Outreach Community Health Centers requiresemployees in certain departments to be fully vaccinated againstCOVID-19, MMR, Varicella and Influenza.
