What You Will Do: The Visit Nurseworks collaboratively with patients, nurses, social workers, physicians,other practitioners, caregivers and community agencies in coordinatingand overseeing the interdisciplinary plan of care for a patient in ahome care/home hospice environment. Ideal candidate possesses excellentinterpersonal communication and negotiation skills along with stronganalytical and computer skills. Prior home health is beneficial.
