As a Community Health Resource Specialist, you'll be making a real impact by:
Establishing trusting relationships with clients and promoting engagement for healthier lifestyles. Facilitating access to social and care support services specifically aligned to identified and prioritized client needs.Assessing clients for health-related social needs and building their capacity to navigate social service systems. Facilitating connections with social service organizations and assistance programs to empower clients.Assisting with healthcare coordination, case management, and system navigation. Encouraging clients to participate in sponsor/purchaser's programs and helping them identify and connect with in-network providers.Addressing barriers to service utilization. Helping clients overcome obstacles to accessing care and promoting understanding and timely utilization of program benefits.Building client healthcare understanding and management of existing conditions. Providing culturally sustaining health education on topics related to health, nutrition, and lifestyle, and facilitating general health promotion and disease prevention education.Supporting self-management and the prevention of health problems. Helping clients take control of their health by promoting self-management and preventive health measures.Documenting client records in a standardized framework. Tracking progress, assessing health-related social needs, and coordinating communication and care within a structured system.Demonstrating up-to-date expertise and providing guidance to others. Developing, executing, and improving action plans, providing expert advice and guidance to others.Advocating for the healthcare needs/perspectives of communities served. Building relationships with community partners capable of servicing the social needs of local populations.Providing and supporting the implementation of business solutions. Identifying business needs, determining and carrying out necessary processes and practices, and monitoring progress and results to drive improvement.Modeling compliance with company policies and procedures. Supporting the company mission, values, and standards of ethics and integrity by incorporating these into the development and implementation of business plans, using the Open Door Policy, and demonstrating how to apply these in executing business processes and practices.
