Job Description/Essential Functions:
Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
We are seeking a qualified Team Leader in our Delaware County location to provide Psychiatric Rehabilitation Services (PRS) in the individual's natural environment: home, school, placeof employment, or the community in general. This position is also responsible for but not limited to the following duties;* To support a PRS team in helping individuals to develop, enhance and retain skills and competencies in living,learning, working and socializing.
To ensure the PRS team helps individuals receive the level of and/or have access to the services/supports theychoose, so that an individual can live in the environment of their choice.
To ensure continuity of services, in compliance with funding sources and Agency regulations.
Provide supervision of individual's record documentation toensure compliance with regulations and to meet fundingsource requirements.
Participate in the resolution of crisis situations; assist withidentifying key individuals and resources that can providedirect support and/or immediate intervention during a crisissituation.
Assist Director with program planning and implementation;provide input into evaluating staff's clinical skills. Mayassist with special projects and reports.
Monitor and ensure staff meets productivity expectations asset by administration, department and funding sources.
Assess initial individual's needs, based on individualstrengths and available resources
Participate in the on-call rotation providing support toindividuals after hours.
Job Qualifications:
PRS Team Leader shall have one of the following:Bachelor's degree and CPRP certification or Bachelors degree and 3 years of mental health direct care experience, with 2 years being work experience in PRS and CPRP certification being attained within 2 years of hire.
Masters Degree with CPRP preferred.
Excellent oral and written communication skills, as well as detail-oriented and able to complete a high volume of documentation in a fast-paced environment.
Strong computer skills are required, as well as Internet research abilities.
Strong analytical and program development skills
Detail oriented and very strong organizational skills.
Knowledge of mental health system and supports.
Must possess a valid driver's license with an acceptable driving record.
Scope:
Frequent talking and listening.
Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include directintervention with individuals.
Possess manual dexterity and fine motor skills.
Must be available for local travel.
May require crisis intervention in emergency situations.
May be required to carry beeper during scheduled emergency on-call rotation.
