Social Worker Details
Essential Job Duties:
Performs home/facility visits to evaluate patient/family in non-hospitalized setting.
Collaborating with the Clinical Care Coordinator (CCC) identifying highly complex cases requiring psychosocial planning interviewing and proceeds as necessary in a timely and effective manner. Makes referrals and follow-up with CCC.
Performs and documents depression screenings and coordinated follow-up as appropriate.
Performs follow-up calls/visits within one business day of care encounter.
Works closely with Medical Management to appropriately interpret/apply beneficiaries benefits and services as resource and liaison to beneficiary and healthcare.
Communicates and collaborates with hospital SW to provide accurate level of transition needs.
Performs psychosocial assessments that are consistently comprehensive and reflect early identification of complex problems or changes in condition that would necessitate revisions to the plan for psychosocial management or healthcare.
Qualifications:
Bachelor’s degree or Master’s degree in Social Work or related field (MSW/MS).
Strong communication skills with excellent verbal and written skills.
Social worker certification by the State of Indiana preferred.
Five (5) years of clinical experience is required within the last seven years.
Strong knowledge of clinical social work and scope of Indiana State laws.
BLS required
Experience in case management strongly preferred
License/Certification:
BSW/MSW preferred
BLS Required
