  Job Description:

  Our Tele Crisis Social Work team works with patients and families throughout the state using our Tele Health platform. As a Tele Crisis worker, you will utilize your clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patient’s crisis assessments, dispositioning patients, providing resources to patients and families, and coordinating care with the onsite physicians and nursing staff.

  The telecrisis team is currently located at LDS Hospital, however, we are expanding to additional locations all over the state. We are currently hiring for our graveyard team which consists of 3-12 hour shifts per week.

  The locations for the telecrisis position include Mckay Dee, Logan, St George and Utah Valley. It’s not remote, but we will set up stations at those hospitals.

This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients. The CSW performs these duties under the direct supervision of an LCSW.

  The locations for the telecrisis position include Mckay Dee, Logan, St George, and Utah Valley. It’s not remote, but we will set up stations at those hospitals.

  Social Worker-Clinical

  The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patients’ significant others, families, healthcare providers, insurers, community resources, and all other involved parties.

  Position Details:

  This is a 24/7 team and, depending on the need, new hires may be scheduled to workdays, nights, or weekends.

  *Salary is based on level of licensure and experience.

  CSW pay range: $29.89-$46.14

  LCSW pay range: $32.89-$50.76

  Scope

  This position provides care appropriate to the established plan of care in the assigned patient population.

  Job Essentials

  Assesses the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment.

  Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in area(s) of responsibility.

  Diagnoses and treats various mental, emotional, and behavioral disorders.

  Performs social work consultation for patients and caregivers. Conducts psychosocial assessments. Provides supportive, crisis bereavement and other social work interventions as necessary.

  Develops a plan of care either individually or with input from the patient, patient family and significant others, physicians, and other members of the healthcare team.

  Modifies care plans to reflect changes in patient status.

  Refers patients to other services to improve their well-being. These referrals may include community services, treatment programs, support groups, psychiatrists, or other mental health providers.

  Educates, communicates, and coordinates with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely discharge, and to identify and resolve delays and issues. Credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.

  May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events.

  Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care teams, outside agencies, and other resources as necessary.

  Works with the multi-disciplinary team to develop a discharge plan to provide further care, services, and referrals to ensure continuity of care. standards.

  Completes timely and accurate documentation in the medical record using knowledge of documentation standards for the department to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing/reimbursement.

  Complies with federal, state, local, and clinical program/facility standards.

  Acts within the scope of the Mental Health Practice Act.

  May identify patients at risk for potential avoidable delays, extended stay, readmission, financial or complex discharge needs. Coordinate with care providers and leaders to reach optimal solutions.

  Minimum Qualifications

  Master’s degree in social work. Education must be obtained from an accredited institution. Degree will be verified.

  Current State Licensure as LCSW or LMFT.

  Preferred Qualifications

  Provides care appropriate to assigned patient population as established with the plan of care/treatment plan and in accordance with professional licensure. This care may take place at a hospital, facility, clinic, inpatient, outpatient, or home care setting.

  Job Essentials

  Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care teams, outside agencies and other resources as necessary.

  May provide psychotherapeutic modalities appropriate to the level of care under the supervision of a licensed clinical social worker.

  Completes timely and accurate documentation in the medical record using knowledge of documentation standards for the department to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing/reimbursement standards.

  May ensure that productivity standards and expectations are met.

  Credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.

  May provide support and debriefing under the supervision of a licensed clinical social worker, in conjunction with the Employee Assistance Program, for staff members following adverse events.

  Provides support and education to patients, families, and staff members on Advance Directives.

  May maintain an active role in denial prevention. Proactively intervenes with payers to prevent inpatient denials. Communicates any necessary information to payers to help appeal existing denials.

  May coordinate the discharge planning process from patient admission to departure. Screens for and assess patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after the hospital stay.

  Works under identified LCSW and actively participates in required supervision through scheduled meetings, consultations, and chart reviews as indicated.

  Minimum Qualifications

  Master’s degree in social work. Education must be obtained from an accredited institution. Degree will be verified.

  Current State Licensure as a Certified Social Worker.

  Certified Social Worker Intern

  Computer skills (email, word processing, and spreadsheets).

  Preferred Qualifications

  Experience working with in a healthcare setting.Note: The Certified Social Worker Intern License can be utilized for up to 24 weeks (6 months) while employees are working toward preparing, testing, and passing the State Boards examination (obtaining an CSW license). After that 24 week period, or if the CSW Graduate fails the State Board exam a second time, the employee can no longer remain in the CSW-I License. These employees can apply for any open jobs they qualify for, including jobs requiring a certification the employee currently holds (i.e. SSW, psych Tec, etc.). Failure to either pass boards or to be hired into another job will result in termination.

  Physical Requirements:

  Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

  Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

  Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

  May be expected to stand in a stationary position for an extended period of time.

  $30.34 - $46.84
