JOB REQUIREMENTS: Duties Independently performs case finding functionsand outreach, identifying and screening patients for psychosocial needs.Independently conducts psychosocial assessments. Incumbent is able tocoordinate community-based services, including information and referralfor additional services from other VA programs, other governmentprograms and community agency programs. Identifies high-risk patientsand provides case management services. Establishes and maintainseffective therapeutic relationships with veterans and their families.Independently provides consultation services to other staff regardingthe psychosocial and mental health needs of veterans and the impact ofpsychosocial and mental health problems on health care and adherence totreatment plan. General knowledge of veterans\' benefits and servicesrelating to special programs, service connected compensation,non-service connected pension. Facilitates referrals based upon veteransneeds and eligibility. Serves on medical center committees and workgroups as needed to support the mission of the organization. Makesrecommendations for treatment or referral in cases where consequencesfor the veteran may have serious effects on current and future lifecircumstances.
