FTE: 1.000000 Shift: 1 Monday-Fridayworking 0800-1630 with 9 weekends and one holiday per year Job Summary:Responsible for coordinating the discharge plan of care for all patientsduring their hospital admission. Coordinates the discharge plan startingon admission and through discharge follow-up. Provides biopsychosocialassessments and provides updates that address the patient\'s evolvingdischarge care needs to timely move them to the appropriate next levelof care. Establishes a plan of care that provides maximum benefit topatients and their families in collaboration with the multidisciplinaryteam in Care Coordination Rounds (CCRs) seven days a week. EXPERIENCEPREFERRED: A minimum of two years experience in an acute care setting ispreferred. Casework in medical social work is preferred. EDUCATIONDESCRIPTION: Master\'s degree in Social Work is required. SPECIAL SKILLSDESCRIPTION: Interpersonal skills necessary to obtain information frompatients, to provide counseling to patients/families, and interacteffectively with patients and families. Analytical skills necessary toassess patients needs, develop associated discharge planning and developsolutions to problems concerning patients mental and physical wellbeing.LICENSURE DESCRIPTION: Current State of Wisconsin license as a socialworker is required.
