Methodist Hospital is looking to hire a Social Work Care Manager to join our Care Integration team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day. Provides direct social work services to patients, extending services to family members in need of identified social worker services, providing them with psychosocial and environmental support. Makes available a full range of treatment modalities with individuals, families and groups, based on social work assessment and diagnosis. Coordinates social work services with other departments and providers within the Park Nicollet Health Services i.e. hospice, hospital and clinic based social workers and out of system providers.
