What you will do:
Assesses the psychosocial status of patients related to the patient's illness and environment.
Carries out social evaluations and plans intervention based on evaluation findings.
Maintains clinical records on all patients referred to social work.
Provides information and referral services for the organization to patients and families/caregivers regarding practical and environmental needs.
Serves as liaison between patients or families/caregivers and community agencies.
Participates in the development of the total plan of care and case conferences as required.
Assists physician and other team members in understanding significant social and emotional factors related to health problems.
Participates in discharge planning.
Other duties as delegated by the Director.
Maintains collaborative relationships with Organization agency personnel to support patient care.
Maintains and develops contracts with public and private agencies as resources for patient and agency personnel.
Provides information to patients or families/caregivers and community agencies.
Participate in department performance improvement activities as assigned and attends required training.
Demonstrates service excellence and ACE.
What you will need:
Graduate of a license program in social work or related field accredited by the Council on Social Work Education and achieved an LSW status.
A minimum of one year's experience in health care.
Experience in a home health care, preferred.
Has good oral and written communication skills; and has good organizational skills.
Possesses and maintains current CPR certification.
Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
Punctual attendance at assigned work location is required.
Ability to work in a safe and efficient manner and maintains an accident free work place, including ability to demonstrates a working knowledge of Medical Center emergency codes.
Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment.
Ability to comply with provisions of applicable S.D.S. forms.
Successful completion of the ninety (90) day probationary period.
