　　About the Role:

　　The School Based Case Manager promotes pro-social behaviors and socialskills to students within a school-based setting. Provides support tothe student to help them achieve academic success. Assist students andfamilies in obtaining recommended service coordination and communitysupport.

　　Responsibilities:

　　- Provide evaluation, treatment, consultation, community education andtraining activities in the school and community.

　　- Involves transportation of self and client in the provision ofservices in a personal vehicle.

　　- Facilitate access and referral to guardians and conservators,self-help groups, advocacy services, and legal advocacy service whenavailable and necessary.

　　- Timely completion of clinical documentation per agency policy toinclude treatment plan document, clinical contact notes, time sheets,managed care authorizations, discharge summary and all others as needed.

　　- Accept cases as assigned by supervisor.

　　- Attend clinical supervision, and team, department and agency staffmeetings as provided.

　　Qualifications:

　　- Bachelor\'s degree in Social Work or Counseling

　　- Previous experience assisting with service coordination and supportsof students and families.

　　- Proficiency with computers, iPads and other related technologies andexperience entering data into electronic medical records (EMR) program.

　　- Must have a valid driver\'s license, reliable transportation withpersonal automobile liability insurance coverage at or above the levelsmandated by the State of Vermont.

　　- *Successful completion of a criminal history background check afterhire.
