Identify and provide for patient and family\'seducational needs. Provide patient care per physician orders and Agencypolicy. Complete all required patient and Agency paperwork in anaccurate, thorough, legible and timely fashion to reflect a clearpicture of care provided. Transcribe, implement and evaluate newphysician orders accurately. Communicate patient\'s issues and concernsto the Case Manager, appropriate health care professional, and / orClinical Manager in a timely and accurate manner.
