Responsibilities:
· The HospiceAdmissions RN is responsible for performing an initial physical, psychosocial,and health systems assessment in order to develop a plan of care, goals, andmake recommendations based on patients’ needs
· The assessment maytake place in hospitals or nursing facilities
· The Access Center RNwill follow the appropriate standards of practice established by the NationalHospice Organization when admitting patients and insure the admission processmeets current regulations
· The RN will alsoengage in community outreach to physicians and other medical providers to shareinformation regarding Hospice and Palliative care, as well as gather feedbackto inform performance improvement initiatives
