POSITION SUMMARY
The Registered Nurse Family Navigator ( RNFN )provides intensive patient-centered clinical care coordination, case management, health education, and culturally-appropriate social support to promote improved health outcomes. The RN-FN may work with pregnant clients with medical or social risk factors for poor health outcomes, assist with other client populations as contracted by Medicaid Managed Care, or as directed by Family Navigator Program manager. Must deliver services in a non-judgmental, compassionate, and professional manner.
PRIMARY ACCOUNTABILITIES :
Direct Patient Care
Acting as a member of the Community Health Programs and Family Navigator team, this Registered Nurse will provide direct health services to a high-risk pregnant patient population.
Utilize the nursing process to improve outcomes and optimize care.
Meet with referred individuals to assess physical and mental health needs to identify and address barriers to care.
Screen, identify, enroll, and provide clients with ongoing intensive RN care coordination services.
Identify individual risk factors and provide appropriate medical and social referrals, facilitating enrollment in eligible programs.
Follow HIPAA regulation and practices to secure and maintain privacy of personal health information. Must practice within the scope of RN licensure as defined by the Ohio Board of Nursing.
Charting and Documentation
Document patient encounters in an appropriate and timely manner in Salesforce and as appropriate, the electronic medical record of clinical collaborators.
Maintain high level of knowledge of contract requirements (e.g., eligibility, scope of work, documentation).
Provide outcome measurements as required by contracts, grants, or research initiatives.
Client Health Education and Advocacy
Provide comprehensive pregnancy care plan, assist in communication to primary care providers.
Provide health education and advocacy to address risk factors and improve adherence to treatment plans to improve health outcomes.
Program Coordination and Outreach
With guidance as needed, write grant proposals, manage grant activities, including data reporting, related to funding used to support duties primary to job description.
Collaborate with community providers and agencies to promote access to and knowledge of navigator services, initiate or participate in service plans, inform providers of changes in pertinent patient social-emotional or health status, access new services, and initiate or participate in service plans for high acuity pregnant clients.
Must be able to provide feedback and updates to providers in a professional manner.
Maintain continuing education specific to diagnoses of population served, and as required by Ohio University.
Minimum Qualifications:
Bachelor’s Degree ( BSN ) with three years of relevant experience.OR
Associate’s degree (RN) with five years of relevant experience.
OH driver’s license
A Registered Nursing license in the state of Ohio is required
American Heart Association Basic Life Support Healthcare Provider certification and willingness to obtain American Heart Association Basic Life Support Healthcare Provider Instructor certification.

Advertised Salary:
The approved hiring range for this role is $51,281.00 to $59,614 . Offers outside of the target hiring range may be subject to additional approvals. The final offer will be commensurate with the candidate’s education and experience in relation to the minimum qualifications for the position, while also considering internal equity. This is a grant-funded position. While additional grants are anticipated, this position is currently funded through 6/30/25.
