About RN- Discharge

  Emergency Room RN Be detail-oriented, inquisitive, and practice a problem-solving approach for ensuring safe discharges for patients seen at Moore ED. Collaborates with nursing, and physician staff and any other ancillary services to resolve conflicting challenges prior to patient discharge.Analyzes nursing clinical documentation from a compliance, regulatory and reimbursement perspective to include initiation, discontinuation and/or adjustment of treatment modalities utilized in the care of the patient. Review LWBS list from last several days and make call backs asking patients how they are doing and offering an opportunity for patients to come back if needed. Escort discharge patients needing wheelchair assistance to their car or waiting room only if ride is coming. At no time should patients be in waiting room without prior discharge transportation, involve case management or charge RN for cab vouchers. Educate discharge ED patients on their conditions including teaching “red flags”. Ie. incapability of medication. Ensuring patients needing DME such as portable oxygen is available. Ensure patients and families understand their follow up care/ appointments if needed. Ensures add on discharge physician orders are completed and discontented. Collaborate with PCC/ED Charge RN or ED case manager for difficult discharges. Demonstrate ability to explain complicated medical terms in plain language for patients and families discharged home and being seen by a provider from the emergency department.Review Moore’s ED EMR’s of patients ready for discharge to ensure the patient is a safe discharge and that there is a review for completeness of documentation. This RN should ensure there is documentation of IV bolus end times, all orders have been completed or reasons why not completed, saline lock removals, repeat stable vital signs and any other red flags associated with the patients presenting chief complaint are documented.Ensures the confidentiality of patient records and promotes patient privacy Valid NC RN License or Compact State RN License required. At least 1 year of ER or Critical Care experience preferred. ACLS, BCLS required. To develop and maintain a highly educated registered nursing staff to meet the present and future demands of the nursing profession, employees who begin employment with FirstHealth of the Carolinas in a position that requires a registered nurse must have a Bachelor of Science in Nursing (BSN) degree within five (5) years of their employment date.
