PeaceHealth is seeking a RN Chronic Disease Navigator for a Full Time, 1.00 FTE, Day position. The general salary range for this job opening at PeaceHealth is $44.25 - 66.37. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.
Hiring bonus and relocation assistance may be available.
Responsible for the coordination of Home and Community patients in collaboration with Medical Directors, Manager Disease Management, attending physician and interdisciplinary team. The RN Chronic Disease Management Navigator will be responsible for monitoring the progress of Home and Community patients throughout all steps of the disease management pathways. Coordination may also include SOC and PRN visits, review and management of care plans, and follow up phone calls. In collaboration with leadership, coordinates implementation of Disease Management educational activities, program evaluation and outcomes research.
DETAILS OF THE POSITION:
Monitor progression of patients throughout the Disease Management Care Pathway to ensure compliance with PH System standards
Conduct phone calls to Home and Community patients to evaluate symptoms and reinforce education
Performs joint visits with home and community staff to provide support and coaching
Compile and review quality outcomes of Home and Community patients
Support continuous improvement efforts that promote clinical excellence, patient safety, and patient experience
Act as subject matter expert resource for colleagues
Ensure regulatory compliance
WHAT YOU BRING:
Graduate from an accredited school of nursing. Bachelor or Masters degree is preferred.
Minimum of 5 years nursing experience is required. 2 or more years of experience in Home Health, Hospice, Palliative Care or other Chronic Disease Management nursing is highly preferred.
If working and licensed in WA, requires Oregon RN license within 3 months of hire. If working and licensed in OR, requires Washington RN license within 3 months of hire
Current BLS certification issued by the American Heart Association or the American Red Cross.
