　　Description:

　　The RN is responsible for admission of patients for home care services, development of the patient plan of care, coordination with patient's provider and other disciplines for the provision of services, and safe delivery of patient care.

　　Collaborates and speaks with providers/physicians to ensure patient care is adequate

　　Documents all relevant information about patient into EMR system

　　Creates own schedule according to patients needs

　　Assess payment and responsible for ongoing authorizations

　　Educates/instructs patients on medical maintenance

　　Care/Discharge plans

　　Skills:

　　Medical, Care management, Patient care, case management, Care plan, EMR, Rn license, Registered Nurse, homecare, EPIC, Home Health, Home Healthcare

　　Additional Skills & Qualifications:

　　Reliability

　　Epic experience preferred, not required

　　Active state of CT RN license

　　1+ years of nursing experience required

　　Home care and skilled nursing experience
