Coordinated Care Services Inc (CCSI) seeks a talented Restoration of Self Sufficiency Facilitator to contribute their expertise in providing essential business services to our provider organizations and customers. The RSSP Facilitator meets with Temporary Assistance Recipients who are unable to work due to a disability. The purpose of the program is to assist clients in navigating community resources and addressing barriers to employment so that they can move towards self sufficiency. CCSI is partnering with Monroe County Department of Human Services (DHS) to hire a RSSP Facilitator. This position is employed by CCSI and working on behalf of Monroe County. In this role, you will be - Conducting initial interviews with clients that Monroe County Department of Human Services (DHS) has identified as being unable to work due to a physical or psychological condition- Working with clients to develop a Restoration to Self Sufficiency Plan that outlines how they can best address barriers to employmentWhen appropriate, connecting clients to community resources like physical or mental health providers, care management, child care or transportation resources- Monitoring attendance at scheduled physical and mental health appointments and following up with clients when they do not attend recommended appointments- Maintaining written and electronic client records- Performing other duties as assignedJob RequirementsHigh School Diploma or equivalency required. Associate Degree in Human Service field preferred with minimum of 2 years of experience in behavioral health field and/or customer service or equivalent education and work experience (see CCSI's equivalency chart).Prior experience in vocational rehab or a general knowledge of community resources would be a plus
