　　Active ME State or Compact Registered Nurse License

　　Valid BLS

　　Graduate of an Accredited School of Nursing

　　Must have Case Management experience in a hospital setting

　　Must have Acute Care discharge planning and care coordination experience

　　Additional certifications may be required
