This position reports to the Nurse Supervisor, and would work between the hours of 7 a.m. and 6 p.m. Monday through Friday and some Saturdays from 8 a.m. to 1 p.m. We are looking for someone who can work 28 to 40 hours per week on a consistent, regular schedule, with some Saturdays hours. A qualified candidate for this position will be hired into one of two job codes based on years of experience. With the exception of years of experience, the required qualifications and job responsibilities will remain the same. BA/BS or a combination of related education and work experience to equal at least four years experience will be hired into a 3521CN (HC Pro 1 - Primary Care Nursing) job code BA/BS plus two years experience or a combination of related education and work experience to equal at least six years experience will be hired into a 3522CN (HC Pro 2 - Primary Care Nursing) job code Essential Functions: Patient Care & Care Coordination 90% Administer care/treatment and care coordination to patients as ordered by medical and mental health providers. Participate and work together with team members to ensure that patients receive whole-person care. Conduct patient and family education related to preventive care, mental health care, chronic care, and condition management as appropriate; connect patients to resources when their conditions are exacerbated. Manage a panel of patients, many with multiple medical and mental health diagnoses, and other patient populations as assigned (e.g. pregnant patients, patients prescribed high-risk psychiatric medications, etc.). Develop patient care plans based on risk and overall patient care complexity. Work with care managers to implement the patient's care plan. Communicate with providers, care coordination team members, and other support staff through the Electronic Health Record (EHR), in person, and by other means. Communicate significant changes in patient status to the provider and care team. Facilitate medication refills and provide medication education for patients as needed. Identify and connect patients to community resources that will assist them in achieving good health outcomes. Follow-up with patients if they have not scheduled their appointments. Check lab/path results from Fairview and if needed call Fairview for results. Follow-up with no-show patients that have not rescheduled. Work with interpreters and/or PSRs to call and schedule non-English speaking patients accordingly. Orient residents and multidisciplinary students as needed. Provide walk-in and phone triage. Address flags, Emergency Department/Inpatient Hospitalization summaries, refill requests. Respond to medical and behavioral emergencies in the clinic. Other Duties as Assigned 10% Attend all-staff meetings, weekly care coordination meetings, weekly nursing meeting meetings, and daily huddles. Other duties as assigned. Job Responsibilities: Apply nursing assessment aptitude and problem solving skills to provide quality care to complex patients Utilize strong time management and organizational skills Utilize knowledge of regulations, policies and procedures for nursing in the primary care setting. Utilize the electronic medical record, registries, or databases to monitor patient outcomes. Must be able to lift 30 lbs. Work in a fast-paced, rapidly changing environment that requires sitting, standing, and walking, with frequently changing priorities. Must be able to be flexible and multitask. Use high-level communications skills to work with patients, staff, providers, and outside agencies. Provide excellent customer service. Work wit patients and staff from diverse cultures, languages, and backgrounds. Work on a team and make positive contributions to the team. This position requires a negative TB test and a Hepatitis B vaccination. Qualifications All required qualifications must be documented on application materials Required Qualifications: BA/BS or a combination of related education and work experience to equal at least four years experience Current licensure as a Registered Nurse in the State of Minnesota. CPR certified. Experience in managing clinical care using an electronic health record. Preferred Qualifications: Two to three years of experience in providing care coordination, skilled nursing or relevant experience. Oral and written proficiency in one of CUHCC's dominant languages: Spanish, Somali, Vietnamese, Hmong, Lao. Experience or certification in mental health nursing.
