Spring Creek Rehab and Healthcare Center is currently looking for a full time Registered Nurse Assessment Coordinator (RNAC) to assist with leading our nursing team! Requirements of the Registered Nurse Assessment Coordinator (RNAC):
Previous experience as a RNAC required
Proficient in MDS
Proficient in PA CMI
Registered Nurse (RN) with effective leadership & assessment skills in a long-term care setting
Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
Responsibilities of the Registered Nurse Assessment Coordinator (RNAC):
Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.
Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.
Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.
Participate in Facility surveys (inspections) made by authorized government agencies.
Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.
Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.
