　　Provides medical evaluations, crisis intervention, and pre-hospitalscreening for consumers.

　　Performs screening for general health with priority for high-riskconditions and chronic illness.

　　Monitors medical charts and provides compassionate care and assistanceto patients and families.

　　Administers medications as prescribed.

　　Establishes rapport with URC consumers and assesses their needs.

　　Provides education on proper medication usage, diet, hygiene, andprevention of chronic disease.

　　OTHER DUTIES AND RESPONSIBILITIES:

　　Coordinates and participates in intensive services treatment team andmakes referrals and recommendations for recovery services.

　　Coordinates services with external and community providers, as well asmonitoring data information input into the clients health record usingHealth Information Technology Systems.

　　Advocates for consumers and community members in need of primary careand/or behavioral health services.

　　Ensures documentation is completed in a timely manner.

　　Complies with all agency policies and procedures, including NorthCaresCode of Ethics.

　　Performs other duties as required and/or assigned by supervisor and/orexecutive staff.
