Description:
The Registered Dietitian responds to all Nutrition referrals generated by EBNHC providers. The Registered Dietitian provides clinical assessments, develops care plans, educates patients and families and provides follow up. The Registered Dietitian also participates in Health Center functions to promote health and wellness and contributes to the management of chronic health conditions in accordance with evidence-based guidelines for medical nutrition therapy. The Registered Dietitian provides individual as well as group counseling. Registered Dietitian’s participate in EBNHC Nutrition Department activities that support clinical excellence. Registered Dietitians are required to maintain both national and state licensures. Clinical competencies are evaluated with the Nutrition Manager.
Duties & Responsibilities
Responsible for responding to all nutrition referrals generated by EBNHC Providers within the context of the Behavioral Health Model of care using Warm Hand Offs and Same Day appointments to meet patient needs.
Demonstrates the ability to review clinical data and develop care plans reflective of the assessment process.
Develops individualized client centered care plans consistent with self-management and dietary goals.
Educates patients and their families as indicated with most updated scientific based nutrition education.
Demonstrates the ability to provide culturally, linguistically and age appropriate nutrition education.
Refers patients to needed services and provides follow up care.
Consults and communicates with other providers as needed and attends meetings as appropriate to promote integration into the Primary Care teams.
Demonstrates the ability to prioritize high-risk patients and situations.
Maintains documentation for all patients in the EMR.
Maintains a knowledge base community and in-house resources.
Works towards collaboration with Health Center Nutrition Staff
Assists in the keeping of statistics for Quality Improvement.
Responsible for continued professional growth and development and maintains credentials. Uses the Academy of Nutrition and Dietetics Professional Portfolio for documentation.
Demonstrated the ability to communicate in an effective and respectful manner.
Participates in the Simmons University Internship community rotation at EBHNC by reviewing projects and providing community and public health nutrition education.
Attends and participates in Journal Club with PACE RD’s.
Adheres to the goals of the C3/ACO by addressing the treatment & prevention of chronic illness with attention to obesity & Diabetes treatment.
Maintains excellent punctuality and attendance.
Maintains a positive attitude and demonstrates commitment to the EBNHC Mission.
Promotes a sense of “team work” through demonstration of self-direction and self-motivation. Solves problems independently and knows when to seek consultation.
Displays outstanding customer service skills.
Adheres to all EBNHC and departmental policies and procedures.
Participates in required departmental activities and meetings.
EDUCATION:
Master’s Degree required.
Registered Dietitian (RD)
Maintains State and National registration/licensure.
Maintains the Academy of Nutrition & Dietetics Professional Portfolio.
EXPERIENCE:
Minimum 1 year as a Registered Dietitian with clinical and public health experience
SKILLS/ABILITIES:
Ability to work in a team environment.
Significant interest and the ability to work within the context of the Chronic Disease Management Model, including patient self management goal setting
Versed in or willing to quickly develop skills in motivational interviewing and concepts of behavior change
Evidence of creativity, flexibility and professionalism.
Ability to work successfully in a peer-to-peer environment.
Bilingual in English/Spanish is a plus.
