External Description
This is a middle level management position responsible for the direct supervision of assigned Community Team Managers/House Managers of IDD Services (CTM/HM of IDD) and the oversight of assigned IDD residences. The Program Specialist of IDD Services (PS of IDD) reports directly to the Associate Executive Director of IDD Services (AED of IDD) of an assigned community. The Program Specialist of IDD Services is a part of the community management team and protects the health, safety, finances, and confidentiality of all Consumers supported by Philadelphia Waiver residential programs.The Program Specialist of IDD Services serves as the liaison between Supports Coordination entities, provider agencies, families, and assigned Consumers ensuring ISPs are accurate, ISP Team Meeting Forms are completed, services are provided, outcomes/goals are addressed, and plans of corrections are implemented. The PS of I/DD also ensures all Consumer services provided and the residences in which the Consumers reside are managed in accordance with regulatory and monitoring standards and policies and procedures. This Program Specialist provides on-going direction, support, and training for Community Team Manager/House Managers/House Managers to ensure they have a clear understanding of their role and responsibility in the ISP process.Additional responsibilities include managing the flow of information between the residences and internal and external departments. The Program Specialist of I/DD Services analyzes various reports pertaining to Consumers' health, safety, and finances as well as staff related functions and implements appropriate plans of action. In the absence of a Community Team Manager/House Manager or vacancy in a Community Team Manager/House Manager position, the Program Specialist, under the supervision of the AED, works collaboratively with the Community Support Team Manager to provide for the continuity of services and staff supervision.The Program Specialist of I/DD Services practices and adheres to NHS's Mission, Vision, and Core Values.Specific responsibilities related to regulatory adherence can be found on the attached checklist.The typical workweek schedule is 8:30 a.m. to 5:00 p.m. Monday to Friday; work hours may vary according to program needs. INDIVIDUAL SUPPORT PLAN
Development:Oversees the following for both residential and day services (if day services is applicable):Compiles Consumer specific data such as medical history, units of service, rates, staffing needs, etc. for all initial/annual pre-ISP AssessmentsCompletes ISP Team Meeting Form along with AED and Executive Director of HCSIS Administration And Capacity ManagementDevelops Consumer outcomes/goals along with AED and Community Team Manager/House ManagerMeets with Supports Coordinator (SC)/Case Manager to transfer ISP dataReviews HCSIS data for thoroughness and accuracyNotifies Executive Director of HCSIS Administration And Capacity Management of any errors and inconsistencies with HCSIS dataChairs internal ISP meetingsCollaborates with Behavior Specialist regarding Behavior Support Plan; submits to Peer Review; works with CTM regarding response and plan of action; monitors adherence to planWorking with Executive Director of HCSIS Administration And Capacity Management and AED, coordinates new admissions to residences and participates in the reassignment of internal Consumer residence movesCompletes initial Consumer Face SheetParticipates in completion of Requisition Form for any change in Consumer status and ensures no change of Consumer status occurs without approval of Executive Director of HCSIS Administration And Capacity ManagementImplementation:Notifies appropriate CTM and Records Department that ISP is available for d wnloadReviews outcomes/goals with Community Team Manager/House Manager in preparation for Community Team Manager/House Manager's DSP trainingServes as a resource for Community Team Manager/House Manager in determining activities in Consumers' immediate neighborhoods in which Consumers can participate to foster inclusionParticipates in the development of Supports Intensity Scale (SIS)Follow up:Analyzes daily, weekly, monthly, progress notes and data collection tools submitted by Community Team Manager/House Managers to verify adherence to ISP; determines any changes needed or inconsistencies in services provided; and initiates changes as appropriateFollowing Community Team Manager/House Manager and SC meeting to review Behavior Support Plan, addresses need for change with written response; follows-up with implementationCompletes Quarterly reviews and any other formal program documentation; updates Face Sheet and Consumer Profile as needed and enters appropriate updates into systemSchedules 90-day Psychotropic Drug Reviews and compiles Psychotropic Review ReportsEnsures Peer Review recommendations are implemented by Community Team Manager/House Manager regarding Consumer Behavior PlansInitiates Consumer burial reserve account; works with Fiscal Department to manage accountWith AED, makes appropriate funeral arrangements
MANAGERIAL/OPERATIONS
Supervision:Supervises assigned Community Team Managers/House ManagersProvides on-going performance review and feedbackConducts monthly supervisory team and 1:1 supervision meetingsApproves PTO requests from Community Team Manager/House ManagersOversees staffing schedules with Community Team Manager/House Managers for all assigned residences to make sure schedules reflect budgeted hoursImplements Progressive Discipline Policy, in accordance with HR policy and practices, directly for Community Team Manager/House Managers/ and as recommended by Community Team Manager/House Managers for DSP'sOperations:Reviews units of service data as needed for accuracy and complianceMeets as needed with Community Team Managers/House Managers to review staff electronic time cards and correct discrepancies via historical edit processDirects emergency procedures i.e. Emergency Evacuation and Relocation Plan, Inclement Weather, etc.
Finance:Oversees Community Team Managers/House Managers management of food and household expenses, staff expenses, and Consumer fundsApproves requests for expenditures of Consumer funds for Consumer-choice/personal needs and Consumer vacationsDistributes petty cash allotment for HCH activities to Community Team Manager/House Managers/House Managers for use at each residenceResponds to alert regarding Consumer SSI funding eligibility and takes appropriate action to maintain Consumer eligibilityRegularly audits Consumer Personal Belongings Inventory
Vehicle:Reports agency vehicle breakdowns to Fleet ManagementImmediately reports vehicle accidents and submits Vehicle Accident Report within 24 hoursFollows up with Incident Report and/or Injury Reporting Packet, when necessary
Documentation:Assesses paperwork submitted by Community Team Manager/House Manager for timeliness, accuracy, completeness and trendsDistributes paperwork to appropriate departments or files accordinglyInputs pertinent Consumer information into Avatar systemTraining:Orients new Community Team Managers/House ManagersReviews Community Team Manager/House Manager training records and
