Job Description:The Clinical Pharmacist demonstrates consistent applicationof knowledge and skills to deliver appropriate, safe, efficient,effective, and cost effective pharmaceutical care in the scope of theirparticular assignment. This position is responsible for the care of patientsas well as the development of effective, professional relationships withinthe multidisciplinary team.
Schedule7 on / 7 off - will include weekends and holidays. Hours 2100-0730 (graves)
Scope
As a Clinical Inpatient Pharmacist, you need to know how to:Operates from a strategic perspective and assumes overall accountability andauthority for respective areas that enhances the overall performance of SCLHealth Pharmacy Services and the organization. Facilitates operationalimprovements, workflow optimization and organizational change throughcollaborative work relationships to enhance performance across the pharmacyteam. Facilitates collaboration to share and implement best practices.Responsible for the implementation of SCL Health Pharmacy Services and SCLHealth Medical Group programs and services.Develops clinical pharmacy programs according to relevant policies andregulations.Reviews records of patients to determine the appropriateness of medicationtherapy, and to identify patients for program outreach. Evaluates patientsconditions and any barriers to medication compliance to ensure all issues arebeing treated. Identifies untreated health problems and refer patients toappropriate physicians or other clinical staff. Develops effective medicationplans that minimize the risk of adverse side-effects.Provides medical staff with appropriate medication information for new andexisting drug therapies to support safe prescribing practices. Assess theresults of pharmaceutical treatments. Collaborates with healthcareprofessionals to ensure optimal patient care.Advises on the correct procurement, storage, and administration of drugsin the clinic setting. Prescribe and discontinue medications, adjustmedication doses, and order labs in accordance with any governingcollaborative practice agreements with authorization from the patientsphysiciaKeep accurate documentation of medication plans and patient progress to meetrequirements for efficient patient care and to support billing practices..
Minimum QualificationsGraduate of an accredited College of Pharmacy with a PharmD. Degree or a five(5) year, required.Licensure by the state of Colorado to practice pharmacy, required.Qualified to practice Drug Therapy Management under the RulesRegulations of the practice state (Colorado), required.Current BLS certification endorsed by the American Heart Association,required.
Preferred Qualifications
Inpatient Pharmacy experience or a PGY-1 ResidencyTwo years of hospital experience preferred as well as the ability to work in afast-paced environment. Additionally, looking for a pharmacist with strongcommunication skills who can respond to codes, think critically, payattention to detail, prioritize, problem solve, adapt to change, serveas the pharmacist in charge overnight, and collaborate with others.
