Description

　　Reporting to the Director of Pharmacy, this position is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients.

　　Required qualifications:

　　PharmD From an Accredited College of Pharmacy.

　　Bachelor's Degree Pharmacy-Acceptable for employees hired prior to the revision date of this Job Description (10/01/12).

　　Upon hire: California Pharmacist Registered

　　California Fire and Life Safety Card - National Organization within 30 days of hire

　　Preferred qualifications:

　　2 years Acute care experience or a Pharmacy Residency equivalent to 3 years of pharmacy practice experience.Why Join Providence?
