About Peer Navigator

  Purpose: Provide a set of services to patients/clients that enable them to empower themselves and develop effective strategies for living healthy lives. Through one-on-one interactions peers provide support and advocacy to patients/clients. This is a great position for a peer that has past HIV/AIDS and or Chronic Illness direct experience.

  Advocacy and Support

  Assist Case Managers during some interventions and assist Health Educator during educational session.

  Through one-on-one interactions with patients/clients, will promote engagement in health care and provide opportunities for education, skill-building, and emotional support in a respectful setting.

  Provide education and information to support and help patient/clients access health information.

  Provide ongoing support and advocacy to patients/clients with the intent to help enhance or develop coping skills, and increase self-determination and self-advocacy that can result in helping improve quality of life.

  Coach patients/clients in self-advocacy and self-management.

  Have an understanding and commitment to core ethical principles and ethics-based boundaries of the work, including appropriate and inappropriate sharing of personal information, and the confidentiality of information gathered during the delivery of peer support services.

  Work in collaboration with supervisors, Case Managers, Health Educators and other members of the interdisciplinary team in order to address concerns related to patient/client needs.

  Schedule appointments on behalf of patients/clients and facilitate appointment-keeping.

  Build relationships with the patients/clients and all members of the interdisciplinary team to support continuity of care and promote health.

  Assist patients/clients with linking them to needed resources in collaboration with Care Management in obtaining and maintaining those services.

  Support patient/client with adherence to treatment recommendations.

  Document each patient/client interaction comprehensively and concisely. All documentation will be submitted to supervisor within the designated time frame.

  Adhere to all government and funder regulations.

  Participates in case conferences, program, and staff meetings when applicable.

  Linkage and RetentionConduct outreach and engagement efforts to engage and retain patients/clients in care.

  Link lost-to-care patients/clients to needed services by providing outreach services to them.

  Coordinate transportation for patients/clients, attend appointments with clients when appropriate, and conduct home visits as needed.

  Promote patient/client independence by assisting them with learning the bus system, scheduling and utilizing medicab and other forms of transportation.

  Work in collaboration with other providers in engaging and retaining patients in care.

  Demonstrate developing knowledge of community resources.

  Employees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits, process improvement and quality improvement plans.

  Qualifications

  High School Diploma or equivalent preferred. Ability to speak and write clearly. Be reflective of the communities/populations being served (bilingual, Persons Living with HIV/AIDS, African American, Latino, LGBT, former substance user, etc.).Be knowledgeable about the region’s services and familiar with navigating the systems of care

  Physical Requirements

  While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
